Services – With 5 bilingual medical and behavioral health clinics, 23 school-based clinic sites, and a satellite geriatric clinic and early learning center, we provide:

Primary medical care
- Behavioral health care
- Substance use disorder services
- Chronic conditions education (i.e., asthma, diabetes, hypertension)
- Social services
- Prenatal case management
- HIV services
- Women, Infant & Children (WIC) Nutrition Program
- Environmental Health and Community Wellness initiatives (lead outreach, habitat restoration, healthy choices programs)
- Oral health care
- Specialty care & PT/OT
Impact - Through our medical and behavioral health care, support services, and community outreach programs, we care for individuals and families who are typically difficult to reach. In 2021:
- 41,545 patients served
- 85.8% identify as Hispanic
- 70% prefer care in a language besides English
- 74.8% live at 100% of the poverty level or below
- 19.3% are uninsured
- 58.8% are on Medicaid
- 42.6% are under 18 years of age
