TITLE: Intensive Case Manager
SUPERVISOR: Guardianship Program Manager/Supervisor of Unsheltered Initiatives
STATUS: EXEMPT
GRADE: THREE
SALARY RANGE: $41,559.44 to $48,184.89
The incumbent functions as an Intensive Case Manager for clients will include but are not limited to linking the clients to community resources to reestablish connections with various social services. The incumbent assists clients in securing basic needs to include, but not limited to: housing, food, shelter, medical and mental health treatment. The incumbent will also assist the client in obtaining any benefits the client is entitled to. Timely and accurate documentation is required and essential in recording the services provided to the client. Work requires tolerance and sensitivity to the needs of the clients. This position requires that the incumbent be on call for their assigned caseload and for street outreach support on a 24-7 basis to include weekends and holidays.
The work environment will include face-to-face contact with clients that are unsheltered and at-risk of having COVID-19 throughout the community and in shelters. It is fast paced, with time sensitive issues and individuals in distress or in extremely difficult situations. The incumbent is expected to handle sensitive situations, maintain a professional demeanor, and a positive attitude in a high-pressure environment.
Knowledge, Skills, and Abilities
Implements all contractual requirements and completes all required documentation in a timely manner to include documentation of all case events.
- Conducts Street Outreach activities which include: (a) engagement; (b) case management; (c) securing basic needs such as clothing, blankets, hygiene items, transportation options, and assistance securing identity-rebuilding documentation such as a Texas ID, birth certificate, social security card, etc.
- Conducts assessments and creates a service plan to identify any resources the client could benefit from and any benefits they might be entitled to.
- Assists clients in accessing other services, including but not limited to food, transportation,
- Medical care, benefits, counseling, and emotional support by locating, contacting, and providing information, referrals, and services to the unsheltered population. For more complex cases, refers clients to agency and/or community-based specialists for assistance.
- Assists in securing safe and sanitary housing solutions such as an emergency shelter and/or a referral for Permanent Supportive Housing and/or Rapid Re-housing.
- Assists in providing transportation options from a place not meant for human habitation or a current temporary residence (Welcome Center, Opportunity Center, Salvation Army, Villa Maria, etc.) to another domicile such as another shelter and/or an apartment.
- Conducts monthly visits, attends appointments, meetings and any other visits deemed necessary to secure basic needs and benefits they may be entitled to.
- Monitors the living condition of assigned clients to ensure that he/she is receiving care, and comfort, and that their basic needs are met, whether the client resides alone, in a foster home, a nursing home or in a more restrictive facility.
- Provides case management to chronically unsheltered individuals.
- Assists in locating and securing an apartment that meets habitability standards and the Fair Market Rate.
- Develop relationships with existing and potential landlords to improve access to housing opportunities.
- Acts as liaison between the client and landlord.
- Advocates for clients' needs and monitors the continuity of care/services.
- Documents case notes for each client and prepares and submits monthly program reports.
- Becomes familiar with the agency's procedural manuals, personnel manual, training materials, contractual requirements, and internal forms necessary to meet the needs of the Intensive Case Manager position.
- Completes other duties as assigned by the Guardianship Program Manager/Supervisor of unsheltered Initiative, COO of Social Services and/or Chief Executive Officer.
Essential Requirements
- BA degree or 4 years of experience in area of delivery of Human Services- Homelessness preferred.
- Familiarity with HMIS/ETO, Rapid Re-Housing and Street Outreach, preferred.
- Strong attention to detail, detail oriented.
- Excellent skills in writing and professional communications.
- Proficient with MS Office and computer software.
- Able to maintain confidentiality working with sensitive information.
- Must have a current driver license and show evidence of meeting state requirements of having the appropriate level of automobile insurance.
- Able to lift and carry up to 30 pounds.
- Must agree to undergo background check by law enforcement as well as a drug test. Results must show that the person has not been convicted of an offense nor have a positive drug test result.
- Bilingual - must demonstrate the ability to communicate in Spanish & English (written & verbal).
- Able to attend training sessions within and outside assigned region and travel to provide services to clients.

Funding for this position is through a grant or contract received by the agency. As with all funded programs, the position is contingent upon continuation of funding.
Evaluation: A review of performance is conducted after a four-month introductory period, and upon completion of twelve months from the date of hire, and every twelve months thereafter.
