Virginia at Home (VaH) offers home-based primary care for older adults who are homebound due to medical, functional, or cognitive issues. The program focuses on patient-centered care, helping participants stay in their preferred environments while reducing unnecessary hospital visits. Services include house calls, caregiver support, and Advance Care Planning.
We are looking for a dedicated, compassionate LPN to join our team. This full-time role involves traveling to patients' homes to perform tasks such as phlebotomy, wound care, and vaccine administration, and assisting the VaH provider team during visits. A reliable vehicle is required for travel, with mileage reimbursed. Responsible for providing direct patient care, ongoing education and reinforcement of care plans, and providing general support in an ambulatory setting under the direction or supervision of the patient's physician or LIP/RN designee in accordance with policy, procedure, and competency to promote patient health and wellness. This position requires providing services to all age populations in a manner that demonstrates an understanding of the functional/developmental age of the individuals served.
