Job Description

Community Support Specialist is an important direct service role within a multi-disciplinary team focused on providing behaviorally-oriented and socially rehabilitative services in the community for clients who are experiencing disruption in functioning due severe, persistent mental illness. We are looking for compassionate advocates and resilient problem-solvers to engage in dynamic work in a community setting.

Services provided focus primary assistance in the domains of physical, environmental, financial, social, occupational, spiritual, intellectual, and emotional well-being. Methods include risk assessment, ongoing assessment of behavioral & psychosocial needs, information & referral, education of client & family, coordination of services with all providers, and assistance with learning necessary skills for successful independence and community integration. CSS functions as part of an interdisciplinary team for client care and works effectively in a multicultural setting. CSS works under the regular supervision and direction of a Qualified Mental Health Professional.
