Position Summary & Mission:

This position provides direct clinical services to clients in the FMHC/FFACT program with approximately 75% of services occurring in the community. Provides counseling, comprehensive case management, and advocacy for FFACT clients, with the goal of reducing recidivism into the Jail, reduce reliance on ER's, and reduce use of both medical and psychiatric inpatient hospitals when appropriate. Assists clients with developing and using natural supports, with obtaining and maintaining stable living situations, obtaining necessary medical care and with the development of healthy and positive coping skills. Attends court with clients on a weekly basis and responds immediately to crisis situations with FFACT clients as necessary. Responsible for rotating on-call phone shifts as assigned.

Essential Functions / Competencies:

Provides effective clinical case management services to improve FFACT clients' ability to live successfully in the community.

  • Performs on-going clinically appropriate assessments of consumers.
  • Develops, with consumer voice, the goals to be addressed in individual treatment plans, tailored care plans, and crisis plans, in accordance with the appropriate model of care.
  • Provides behavioral counseling, crisis and stabilization services to consumers.
  • Maintains professional clinical boundaries with consumers.
  • Provides wrap-around case management and referrals as clinically indicated.
  • Provides advocacy with the court system, the jail, and other community supports as needed.
  • Maintains clinical competency and resource knowledge required to meet changing consumer, community, and industry trends.
  • Completes written reports for the court and attend scheduled court review hearings with clients on a weekly basis.
  • Attends treatment planning meetings and daily team meetings.
  • Assures confidentiality is always maintained.

Completes clinical documentation in a timely, accurate manner.

  • Develops individualized Recovery Plans with each client, including measurable goals and objectives for recovery and wellness, with a focus on reducing recidivism.
  • Reviews and updates Recovery Plans with each client as frequently as needed, but at a minimum every 180 days.
  • If a Crisis Plan is needed, formulates it with the client, addressing all required elements.
  • Completes Discharge Summaries in a timely manner, as clinically appropriate.

Completes progress notes on time for each service providing a clear, concise description of the client's progress towards treatment goals, being sure to document any risk concerns.

  • Current YTD progress note compliance percentage (will be calculated for PR).
  • Progress notes routinely meet medical necessity requirements.

Provides Secondary and/or Optional Therapist functions effectively.

  • Accompanies and supports co-workers on outreaches.
  • Provides effective crisis and stabilization services as needed.
  • Develops and/or facilitates effective groups.
  • Serves on and contributes to department or agency committees as needed.
  • Attends all Program and Agency mandated trainings and meetings.

MA Level therapists may be trained and scheduled to provide intake assessments.

Completes administrative documentation in a timely and accurate manner.

  • Completes and submits Time Sheets, Time-Off Requests, Mileage Reimbursement forms, etc. accurately and on time.
  • Provides all required annual documentation to Human Potena3Q1 and completes all required Relias Learning training in a timely manner.

Job Scope: Clinicians work semi-independently managing a caseload of consumers in a highly competent and professional manner. Work involves a significant variety of non-routine tasks of moderate complexity which are guided by formal policies and general procedures. While this position has limited direct budgetary accountability, errors in work can have significant impact on customer service, clinical outcomes, and liability implications.

Communication / Customer Contacts: This position involves a high frequency of communication, in various forms, verbal and written, with clients, other staff, and community partners. Must be able to successfully navigate different systems and partners, with the goal of promoting success for FFACT clients. Must also abide by all confidentiality laws and pertinent codes of conduct.

Knowledge and Skills: A knowledge and understanding of mental disorders; familiarity with the DSM-V (and competency in accurate diagnoses for MA Therapist); knowledge of the major components of a mental status examination and competency in performing on-going assessments; ability to implement a continuum of care for consumers based on consumer need and level of functioning; ability to develop and implement treatment plans; a basic knowledge of psychotropic medications with the ability to recognize side-effects; sound clinical skills and a high degree of professionalism; a basic awareness of substance abuse/dependence issues; a thorough knowledge of community resources and the ability to develop appropriate links for consumers; well-developed skills in managing time, priorities, and paperwork; a sensitive perception of the needs of individuals with disabilities; a keen understanding of the importance of clear, professional boundaries, and maintenance of such; possession of proven teamwork skills; excellent written and oral communication skills; and demonstration of good common sense. The ability to work effectively in a team environment and to collaborate effectively with external partners is an absolute must.

Job Conditions Must be able and willing to drive a car both in cities and on highways; possess a good driving record; be able to work both in and out of the office and go into consumers' residences, which may require climbing stairs and walking on uneven terrain.

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $30.40 - $43.75 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
