Location Address: 8100 Constitution Pl NE Albuquerque, NM 87110-7643 Type of Opportunity: Part time FTE: 0.80Summary: Under the supervision of Complete Care physicians, provides Primary Care, Urgent Care and Hospital at Home care to patients seen in the program. Complete Care serves patients have been referred due to their medical fragility and complicated conditions. Patients and family members will need well-coordinated medical care concordant with their personal goals for care. Work will involve direct medical management as well as collaborating with the physicians, pharmacists, nurse case managers, care coordinators, behavioral health specialists and other consulting physicians and other healthcare professionals who comprise the patients care team. Overseeing any care transitions to ensure high quality care will be a component of the work. Patients may be seen in many care settings, including home, clinic, assisted living and nursing home.Job Description: Responsibilities:
Additional Job Description: Masters degree in Nursing, New Mexico Nurse Practitioner license, federal and state DEA license and ANCC certification required.
