Peachford Hospital is a 246-bed, acute care psychiatric hospital located in the well-established North Atlanta Dunwoody neighborhood. We feature individual units for children, adolescents, adults, and seniors, and offer inpatient acute care, partial hospitalization, virtual intensive outpatient programs and ECT. Nearly 10,000 patients receive compassionate care from our health care team each year and we have been providing compassionate care since 1973. As we continue to evolve, we have created a new position for a Patient Care Navigator.

The purpose of the Patient Care Navigator is to ensure communication and coordination of care among members of the multi-disciplinary treatment team. This may include discharge planning, family contact, safety planning and issue resolution for our patients. This position will work across all services lines and patient populations. The Patient Care Navigator may meet with all patients and their families to plan and discuss discharge needs including the benefits of stepdown services as part of the continuum of care. The positions works cooperatively as a member of a multi-disciplinary team and will meet with the patient's assigned social worker/case manager, charge nurse, physician, step-down coordinator and utilization review team to ensure the highest level of care and patient satisfaction.

RESPONSIBILITIES:

  • Provide navigation for patient and families by addressing any concerns and providing insight to the continuum of care.
  • Assist patient and families in connecting with necessary supportive after-care services, psychosocial care, and social work.
  • Maintain regular communication and updates with and to the patient's providers.
  • Refer to internal or external case management services when other issues are identified (i.e. hunger/homelessness issues, domestic violence issues, etc.)
  • Provide advocacy, patient education and support in accessing community-based and hospital-based programs.

Qualifications

REQUIRED EDUCATION: Master's degree in behavioral health related field preferred.

REQUIRED EXPERIENCE: At least three years of behavioral health experience. Prior social work, case management or family therapy experience preferred.

LICENSURE/CERTIFICATION: LPC-A, LPC, LMSW, or LCSW preferred.
