Transitions of Care Expeditor - MSW

The System-wide Transitions of Care (TOC) Expeditor oversees efficient patient throughput, identifies barriers to timely patient care, and proactively recognizes negative trends to be reviewed. This role is a critical catalyst to continuously improve the care of our system's patients so they can receive their treatment quickly to help them on their road to recovery. This role maintains situational awareness through communication and available technology.


Applies knowledge of healthcare systems, resources, and patient needs to guide patients through their medical journey, ensuring access to necessary services and support.
Expedites, prioritizes, synchronizes and mediates (in real time) clinical issues, removes barriers to discharge and addresses delays in care; identifies potential issues so staff can prioritize patient-care activities and discharges and mitigate potential bottlenecks before they occur.
Confers with multi-disciplinary teams to identify effective strategies/approaches to reduce patient length of stay, handle complex discharges, conditional discharge orders and related issues ensuring patients have a safe discharge.
Performs other duties as assigned.

EDUCATION:
Required: Bachelor of Nursing (BSN) or Master?s in Social Work

EXPERIENCE:
Required: 3 years of recent care management or social work experience

LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Required: Registered Nurse license in North Carolina or a Compact state or Compact state Licensed Clinical Social Worker
BLS (CPR)-American Red Cross or AHA Healthcare Provider
