At Healthcare in Action, we serve patients experiencing homelessness through an innovative "street medicine" approach by contracting with Medicare and Medicaid health plans as well as hospitals and health systems. Street medicine entails using a medically equipped van to deliver medical care directly to people experiencing homelessness in locations such as parking lots, encampments, and parks. The Peer Navigator supports members who are experiencing/ are at risk of experiencing homeless with managing their own health in theDowntown LA/West Hollywood/Marina Del Rey, CA Area. Assist in motivating behavioral changes in patients to improve health outcomes through education, peer support, housing & resource navigation, and the relaying of shared experience. HIA was established in 2021 as a 501 (c)3 organization and associated medical group. Utilizing mobile clinics staffed by healthcare professionals, community health workers, and social workers, our teams travel directly to unhoused individuals living in communities across California, providing services where they reside - including tents, cars, shelters and temporary housing. In meeting patients where they are and supporting their goals, HIA team members build trust - a critical component that can lead to meaningful and positive changes in health and housing. The Senior Director, General Manager reports to the Chief of Integrated Care Delivery and is responsible for the overall growth, productivity, operational and financial performance, and culture of a region. In this high-impact role, you will drive key initiatives that promote sustainable regional growth, foster partnerships with government agencies and other care delivery organizations and play a vital role in the development and implementation of new service lines. This position serves as a critical partner to senior leadership and collaborates across functional teams to achieve both growth and operational excellence. Responsibilities include:
- Strategic Planning & Execution * Develop and execute regional business strategies aligned with HIA's organization-wide goals. Utilize their knowledge, experience, and relationships to identify, evaluate, and present regional strategic growth and development opportunities to the Chief of Integrated Care Delivery and the Chief Executive Officer. * Direct strategic projects and service lines that enhance regional growth and maintain clinical operational excellence. Collaborate with the VP of Medical Services to foster a culture of continuous quality improvement and ensure HIA drives industry leading quality care for people experiencing homelessness in all HIA service lines. * Identify and prioritize opportunities to drive growth, quality, and patient satisfaction.
- Operational Oversight * Direct day-to-day operations across teams to ensure efficient, compliant and trauma informed care in all HIA service lines including but not limited to clinical and social services. * Review reports on performance and quality metrics for staff. * Direct workflows and collaborate on process improvement initiatives to enhance operational efficiency.
- Regulatory & Compliance * Ensure compliance with all healthcare regulations, reporting requirements, and audit activities. * Partner with compliance and legal teams to support regulatory submissions, filings, and corrective action plans. * Act as a subject matter expert of changes in healthcare delivery regulations.
- Cross-Functional Leadership * Serve as a liaison between senior executive leadership and operational teams. * Collaborate across functional areas, including Clinical Leadership (Lead Physicians, Advanced Practice Clinicians, and Physician Supervisors), Operations, Finance, Compliance, Provider Network, Clinical Programs, to implement and drive organizational priorities. * Champion the delivery of integrated, patient-centered care by fostering coordination and collaboration among multidisciplinary teams, ensuring best practices in team-based care models are adopted and consistently applied. Influence and align internal stakeholders to drive shared goals and accountability.
- Performance Management * Make informed decision-making and business recommendations. * Prepare budget planning and forecasting for your region. * Direct contract negotiations and business partnership oversight related to regional services.
- Act as the single accountable point of contact for all accounts, grants and contracts in a region. Maintain in depth and detailed knowledge and understanding, as the accountable representative for all partnerships and cultivate positive relationships with the external representatives. Provide operational oversight of the staff, documentation, reporting requirements, renegotiation timeframes, and billable workflows necessary to successfully execute the contracts and partnerships.
- Monitor and maintain team productivity metrics and the budget performance of a region. Achieve effective results by clearly communicating expectations, monitoring and appraising job performance, motivating, coaching and counseling, developing opportunities for career growth, and initiating and enforcing policies and procedures.
- Manage all hiring and staffing issues and staff development working in partnership with HR. Act as accountable regional leader engaged, aware, and knowledgeable about employee performance issues across service lines in your designed region and contracts. Ensure that regional managers are appropriately and effectively coaching, documenting, and escalating HR and employee performance issues to your leadership and SCAN HR. Identify gaps in cultural leadership and team building and appropriate utilization of SCAN in-person and virtual trainings to develop a team with cohesion and unity.
- Accountable leader for regional strategic growth initiatives with a specific focus towards existing contracts and new expansion opportunities aligned with HIA and SCAN's mission and values to serve PEH, street medicine, and comprehensive integrated care and existing service lines.
- Develop and drive HIA organizational culture; influence through empowerment of clinical and operational teams, creation of project work plans, status reports, maintenance of risks/issues/decisions logs, etc.

Qualifications and experience:
- Strong preference for an MD, DO, Nurse Practitioner or Physician Assistant.
- 10+ years of progressive experience in healthcare, with at least 5 years in clinical operations, managed care, or patient care delivery.
- Bachelor's Degree or equivalent experience in Healthcare Administration, Business, Public Health, or related field preferred
- Expertise with Federal and State regulations, including managed care processes and contracts
- Demonstrated ability to negotiate with Sr. Leadership of Medicare/Medicaid health plans and other entities.
- In-depth experience with operationalizing direct patient care delivery systems, and Quality Improvement strategies.
- Experience in working in a multi-disciplinary healthcare setting with a variety of clinical service offerings and the coordination of patient care.
- Familiarity with data analytics platforms and reporting tools.
- Demonstrated experience with budgeting/forecasting and P&L
- Subject matter expert in healthcare delivery regulations and any changes as they occur
- Demonstrated leadership/supervisory experience
- Ability to engage in clinical supervision and direct patient care as needed
- Must be based in the greater Los Angeles area or willing to relocate.
- Prior experience in a health plan, ACO, or value-based care environment.
