Facilitate the crisis/exception process with people supported/and their families, affiliates and Kansas Department of Aging and Disability (KDADS) staff
• Participate/facilitate complex needs situations with people supported and their families, Targeted Case Managers (TCM's), Managed Care Organizations (MCO) and KDADS staff.
• Assist Access Coordinator with transfers in/out, gatekeeping and Child in need of Care (CINC) cases.
• Complete functional assessments for special re-evaluations
• Assist with managing access in the new waiver (community support waiver)

• High school diploma or equivalent
• Bachelor's degree in Human Service or related field
• 5 years experience in IDD Field

Experience may be substituted for degree
