Merakey is seeking ab Intellectual and Developmental Disabilities (IDD) Incident Reporting Specialist to join our IDD division in Hummelstown, PA.



Earn up to $22.17/hour with the selection of enhanced pay. Base rate of $20.17/hour.



Position Details


The Incident Manager is responsible for/or assisting in the reporting, follow up and facilitating reporting and oversight/collection of administrative review and follow up for all reportable incidents as outlined in Incident Management Policy. Additionally, the Incident Manager will complete reports, track incidents internal and reportable, as directed by administrator or appropriate designee.





  • Ensure timely and accurate reporting, tracking, and follow-up of all internal and reportable incidents per Merakey AVS policies and state regulations.



  • Coordinate with the Quality Compliance Office (QCO) to complete the EIM process, including notifications, investigator requests, and documentation.



  • Notify families or legal guardians promptly and ensure individuals receive necessary medical or emotional support following reportable incidents.



  • Maintain clear communication with the Senior Director or designee regarding incident status, required actions, and compliance with standards.



  • Finalize incidents and ensure all follow-up actions are completed within required timeframes.



  • Support Department of Health surveys, participate in relevant meetings and investigations, and maintain incident tracking systems.



  • Promote individual health, safety, dignity, and rights; attend related committees and perform additional duties as needed.
