The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. We are deeply committed to our clients and are dedicated to implementing a comprehensive model of care. The Guidance Center (TGC) is currently seeking a NFMH Team Lead. This position provides leadership and direct service coordination within a Nursing Facility for Mental Health, supporting residents with serious mental illness and complex behavioral health needs. This role is responsible for delivering targeted case management and general case management services, ensuring individualized care planning, service linkage, and advocacy. Evenings are required (1+days per week after 5 PM). Benefits

EDUCATION AND LICENSURE: Required: Bachelor's degree required. Degree in Social Work, Psychology, or human service field preferred. Equivalent work experience in the human services field, or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education, may be considered. EXPERIENCE: Required: Three years of case management experience with a bachelor's degree. In lieu of degree, a minimum of seven years of case management experience is required. Current internal applicants must be at the Case Manager II level.
