The Guidance Center (TGC) is the Community Mental Health Center and Certified Behavioral Health Clinic serving Leavenworth, Atchison, and Jefferson counties in Kansas. The Guidance Center (TGC) is currently seeking a NFMH Team Lead. This position provides leadership and direct service coordination within a Nursing Facility for Mental Health, supporting residents with serious mental illness and complex behavioral health needs. This role is responsible for delivering targeted case management and general case management services, ensuring individualized care planning, service linkage, and advocacy. Evenings are required (1+days per week after 5 PM).

Responsibilities



  1. Provide administrative support to ensure smooth daily operations of the NFMH specialized services.



  1. Determine, Develop, and Disseminate Logistical Support in the Implementation of Enhanced Care Options as Based on State Prescription or Client Need.



  1. Maintain, Refine, and Promote Collaborative Communication and Service Implementation Transparency between All Stakeholders abiding to the Olmstead Agreement including Disability Rights Center, Valley, KDADS/KDHE, Olmstead Navigator.



  1. Assign Cases and Delegate Staffing in Support of Service Delivery to Specialized Service Recipients following CSS Director Staffing and Approval



  1. Deliver targeted case management (TCM) and general case management services to residents in the Nursing Facility for Mental Health.



  1. Conduct assessments, develop individualized service plans, and coordinate care with internal and external providers.



  1. Monitor resident progress and adjust service plans to reflect changing needs and goals.



  1. Facilitate interdisciplinary team meetings and collaborate with nursing, clinical, and support staff.



  1. Advocate for resident needs and support transitions to less restrictive environments when appropriate.
  2. Ensure compliance with state and federal regulations governing NFMH facilities.


EDUCATION AND LICENSURE:

Required: Bachelor's degree required. Degree in Social Work, Psychology, or human service field preferred. Equivalent work experience in the human services field, or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education, may be considered.

EXPERIENCE:

Required: Three years of case management experience with a bachelor's degree. In lieu of degree, a minimum of seven years of case management experience is required. Current internal applicants must be at the Case Manager II level.

Preferred: Experience with the Kansas system of community-based mental health care and associated evidence-based practices
