Overview

The Hospice Clinical Director of After Hours and Crisis Care is responsible for overseeing the clinical operations of the after-hours teams, including triage and field; as well as Nurse Practitioners and Crisis Care, ensuring that patients receive high-quality care in accordance with hospice standards and regulations. They are responsible for the overall delivery of quality hospice service to patients and families, in congruence with rules and regulations. They act as a champion of education, training, and delivery of quality care after hours and during times of Crisis Care and are a resource that provides guidance and support to their respective teams. They are a critical part of team manager orientation and ongoing education, growth and retention. In this role, they work side-by-side with the clinical and operational team to solidify the highest quality clinical patient care.

SCHEDULE:

This is a hybrid role, that acts as a liaison between the day leaders and After-Hours team.

Flexible work hours are required in this role to effectively lead the After Hours teams.

* Collaborates closely with VNS Health Call Center Team to provide for increased patient experience

* Supports the Managers, Evening/Night/Weekend by providing backup services to team and assists with responding to phone calls and/or providing after hour support when necessary.

* Develops, refines and implements guidelines and standards with Interdisciplinary Group (IDG) for after hours care. Monitors and ensures staff adherence to guidelines/standards.

* Analyzes and resolves complex clinical and financial case management issues in collaboration with Operations, Compliance, Legal, and Finance departments.

* Collaborates with leadership to develop strategies designed to improve financial and clinical performances of the After Hours programs.

* Promotes utilization of Crisis Care to both internal and external customers. Analyzes utilization and effective deployment of Crisis Care staff to assure patient needs are met. Collaborates with clinical management to ensure documentation justifies Crisis Care Level of Care.

* Oversees competencies of After Hours and CC staffs to assure family satisfaction with service and works collaboratively with Education Manager and Hospice After-Hour Managers and Nurse Practitioners to optimize quality of care.

* Participates in Hospice Leadership meetings, and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. * Understands, implements, and complies with Hospice Care Mission, objectives, policies and procedures to ensure patient/caregiver needs are met.

* Performs all duties inherent a senior managerial role. Approves staff training, hiring, promotions, terminations and salary actions. Prepares and ensures adherence to the department budget.

* Participates in special projects and performs other duties as assigned.


Qualifications

Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required

Certification in Hospice and Palliative Care preferred

Licensed and currently registered to practice as Nurse Practitioner in New York State and DEA license preferred

Education:
Bachelor's Degree in Nursing from an accredited program required Master's Degree in Nursing preferred

Work Experience:
Minimum eight years of progressively responsible managerial experience in a triage center, hospital, health care or clinical setting required

Minimum five years of experience in a Hospice Program required Minimum of five years progressively responsible experience working a RN required, preferably in a Hospice environment

Effective oral, written and interpersonal communication skills required

Position requires flexible hours and ability to travel to in person meetings as needed


Pay Range

USD $122,300.00 - USD $164,000.00 /Yr.
