Overview

We are seeking an LPN Care Manager, also referred to as a Licensed Practical Nurse Care Manager or LPN Clinical Care Manager. An LPN Care Manager in our hospital assesses patient needs and helps coordinate care plans with the healthcare team. They monitor patient progress, facilitate communication, and assist with discharge planning to ensure smooth transitions. The LPN Care Manger position supports efficient patient care and resource use, often focusing on patients with stable or less complex conditions.

Shift's Available:Days

Employment Type: Full Time

Hours: 8:00am-5:00pm (8-hour shifts)

Location: Providence Medical Center - Kansas City, KS

Travel: Travel between two clinics in Kanas City, Kansas area

Responsibilities

Essential Duties and Responsibilities (includes, but not limited to):

  • Assessing patient needs: conduct initial and ongoing assessments of patients' health status to determine care requirements
  • Coordinating care plans: collaborate with doctors, nurses, social workers, and other healthcare professionals to develop and implement individualized care plans
  • Monitoring patient progress: track patients' response to treatment and adjust care plans as needed
  • Facilitating communication: act as a liaison between patients, families, and the healthcare team to ensure clear communication and understanding of care goals
  • Discharge planning: prepare patients for discharge by arranging follow-up care, home health services, or referrals to other facilities
  • Teamwork: Support physicians and patients by managing appointments, coordinating care, monitoring patient goals, documenting health status, and facilitating communication to improve clinic services and reduce hospitalizations


Qualifications

Before we go any further, we do have some deal-breakers. You must have:

  • Current state LPN License as required
  • A minimum of Five years primary care practice experience with adult medicine and pediatric patients in a primary care/family practice setting

Additional Required Knowledge, Skills & Abilities:

  • Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk, and patient education

Additional Qualifications That Are a Plus:

  • Care management experience preferred
  • Experience as participant in continuous quality improvement preferred
  • Completion of self-management support training preferred
