Job Summary

#CA-DS

The Solid Organ Transplant Pharmacist hybrid position focuses on patient care within both the inpatient and ambulatory care spaces. This Clinical Pharmacist works with the kidney, liver, and heart transplant teams.

The Clinical Pharmacist is responsible for directing and maintaining drug therapy across the continuum of care, providing comprehensive service-based monitoring on select services, as well as focused monitoring in the areas of supportive care/symptom management, antimicrobial management, anticoagulation, and parenteral nutrition therapy. The Clinical Pharmacist continuously seeks out interventions and offers suggestions on individualized medication regimens.

The Pharmacist is an integral member of the health care team, also contributing to drug distribution activities, providing drug information, serving as a preceptor to pharmacy students and residents, conducting clinical research, and participating in department and hospital committees, task forces and work groups. They foster excellent interdepartmental relationships and provide quality and cost-effective pharmaceutical care to all patients, employees and medical staff of UCDMC.

Minimum Qualifications - For full consideration, applicants are encouraged to upload license and/or certification if required of the position.

  • California Pharmacist License eligible and possess a valid license at the time of hire
  • American Heart Association (AHA) BLS CPR or UCDH-issued CPR Certification
  • Doctor of Pharmacy Degree from an accredited college or university, or equivalent experience/training.
  • Completion of Pharmacy Practice Post Graduate Year 2 (PGY2) in Solid Organ Transplant, or 5 years of Pharmacist experience in similar area
  • Computer skills that facilitate clinical patient data medication order entry and other basic clinical software programs.
  • Demonstrated ability to effectively communicate with and educate patients about their medication therapy.
  • Written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English.
  • Knowledge of current ambulatory care guidelines (i.e. NCEP, JNC, ADA).Demonstrated, ability to identify medication-related problems in patients, communicate them to a patient's physician and offer recommendations.
  • Complete working knowledge of, and skill to apply, practices, principles and techniques of patient care related to area of expertise.
  • Decision-making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached.

Preferred Qualifications

  • Pharmacist Board Certification [e.g., Board Certified Transplant Pharmacist (BCTXP), Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Ambulatory Care Pharmacist (BCACP), or Certified Specialty Pharmacist (CSP)].
  • Experience working with and directing residents, students, and health care professionals.
  • Problem-solving skills to independently recognize and define a problem; identify the resources available to help solve the problem; create and implement viable solutions; and follow through to ensure the problem is resolved to the satisfaction of all parties.
  • Ability to supervise technicians and establish priorities of workflow.

The Department of Pharmacy is a dynamic, integral, and critical part of patient care provided by UC Davis Health. As a department of greater than 600 professional, technical, and administrative support personnel, we focus on providing patient-centered medication management that optimizes outcomes through practice, education, research, and leadership through the continuum of care. We also have the largest pharmacy residency program in the country with 46 total residents in 13 different programs.

The Department of Pharmacy offers comprehensive pharmaceutical services (unit dose, IV admixtures, clinical services, infusion pumps, and teaching) throughout the health system complex 24 hours a day, 7 days a week. The pharmacy services encompass inpatient pharmacy services as well as an expansive footprint in the ambulatory care space where we have clinical pharmacists embedded within our clinics and specialty care locations. Our operational teams support the health system enterprise with three inpatient pharmacies, five outpatient pharmacies, two infusion pharmacies, a home infusion service line, and the full complement of pharmacy purchasing, stores, and distribution.
