Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Orange Coast Medical Center. The employee shall support Orange Coast Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities.
The Care Coordination Supervisor oversees daily operations for Care Coordination and Discharge Planning, ensuring compliance with all regulatory and payer contract requirements. As a front-line leader, this individual is accountable for the supervision of licensed and unlicensed department staff, including but not limited to recruitment, development, retention and discipline. Responsibilities include performing audits and projects for the department, in addition to handling a care management caseload when needed due to staffing needs.
The individual reports directly to the Manager of Care Coordination, who provides direct oversight and direction for both operational and clinical service needs.
The Care Coordination Supervisor collaborates with other multidisciplinary disciplines to ensure safe and effective patient care delivery and the achievement of desired outcomes. This individual is responsible for the facilitation of appropriate length of stay and continuum of care outcomes, through the care management of all systems and services required for an organized, multidisciplinary team approach to care. The Care Coordination Supervisor uses a problem-solving process that includes assessment, problem identification, goal definition, plan development, monitoring, evaluation, reassessment, and revision of the plan, and documentation in the patient record to achieve optimum patient outcomes on both an individual and population basis.

Essential Functions and Responsibilities of the Job

1. Supervision of licensed and unlicensed department staff, including but not limited to overseeing payroll, ensuring optimal staff scheduling monthly, along with daily assignments, handling sick calls, participating in recruitment and retention of case managers and discharge planners, conducting employee reviews and imposing discipline.
2. Leads and/or participates in the planning, directing, and implementation of assigned projects.
3. Encourages patient-focused care through multidisciplinary rounding and ELOS (Outlier) rounds as assigned by the Manager of Care Coordination.
4. Assists in the development, monitoring and sustainment of budget requirements.
5. Works with staff to manage patient flow.

6. Under the leadership of the Manager of Care Coordination, confidently conducts audits to ensure full compliance with regulatory standards.
7. Takes on rotational on-call duties and provides Manager relief as needed, including on weekends or as directed by the Manager of Care Coordination.
8. Responds to and resolves difficult and sensitive patient care issues and customer inquiries and/or complaints.
9. Participates in activities to maintain magnet status.
10. Responsible for taking patient load assignments for department coverage as needed.
11. Ability to be at work and be on time.
12. Ability to follow company policies, procedures and directives.
13. Ability to interact in a positive and constructive manner.
14. Ability to prioritize and multitask.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications.
