Post-Discharge Clinic APP Job Description - 0.3 FTE University of Missouri Health Care seeks an innovative and transformational advanced practice practitioner to provide exemplary care to medical patients recently discharged from the hospitals and to improve transitions of care for discharged patients. The Post-Discharge clinic aims to improve patient's clinical outcomes by ensuring seamless flow as they are discharged from the hospital and connecting to their post-discharge clinical and community resources. The Post-Discharge Clinic acts as a bridge clinic to ensure recently hospitalized patients receive multi-disciplinary support to prevent unnecessary ED visits or readmissions.
