Post-Discharge Clinic APP Job Description - 0.3 FTE

University of Missouri Health Care seeks an innovative and transformational advanced practice practitioner to provide exemplary care to medical patients recently discharged from the hospitals and to improve transitions of care for discharged patients. The Post-Discharge clinic aims to improve patient's clinical outcomes by ensuring seamless flow as they are discharged from the hospital and connecting to their post-discharge clinical and community resources. The Post-Discharge Clinic acts as a bridge clinic to ensure recently hospitalized patients receive multi-disciplinary support to prevent unnecessary ED visits or readmissions.

The primary responsibilities include
- Work independently and in collaboration with physicians and other health care professionals to provide care to recently discharged medical patients
-Provide post-discharge patient care through telehealth visits for high-risk patients, including completing a medication reconciliation, reviewing hospital test results, monitoring symptoms, and ensuring appropriate referrals and appointments are scheduled
-Collaborate with a multi-disciplinary team, including Case Management, Pharmacy, Nursing, Physicians, Nurse Navigators, to ensure optimal patient outcomes
-Assist with handoff of patients to a Primary Care Physician for longitudinal care
-Participate in system-level projects to improve transitions of care

Infectious Diseases (ID) APP Job Description - 0.7 FTE

The primary responsibilities include:
- Work collaboratively with physicians in the Division of Infectious Diseases and assist in a multidisciplinary practice for the treatment of ID-related diagnosis in inpatient and outpatient settings, with focus in our OPAT (outpatient parenteral antimicrobial therapy) and orthopedic ID programs.
- Perform essential patient care functions, which support and expand ID care practice.
- Assist in the organization and clinical processes of ID care.
- Diagnose and treat infectious diseases in the ID clinics.
- Assist in the evaluation and assessment of potential infectious diseases.
- Participate in Telemedicine as an extension of ID care and outreach.
- Obtain and document a complete and focused history and physical and be able to relate other aspects of medical information to the physicians.
- Conduct comprehensive health assessments and demonstrate a high level of autonomy in the diagnosis and treatment of actual and potential health problems.
- Assists in the identification of trends related to patients and participates in quality improvement activities.
- Help guide the development and implementation of structure, process, and outcomes standards.
- May participate in clinical research, as needed.

Shift Type: Monday through Friday 8am - 5pm

Minimum Qualifications

  • Physicians Assistant (PA) licensure by the Missouri State Board of Registration for the Healing Arts.
Anticipated Hiring Range

Hiring Range: $90,251.20-$115,000.00 yearly
