Hourly Pay Range:

$30.46 - $45.69 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Social Worker - Neurology/Memory- Days

Position Highlights:

  • Position: Social Worker
  • Location: Swedish Hospital (Chicago)
  • Full Time/Part Time: Full time
  • Hours: Monday-Friday, 8am - 5om
  • Required Travel: Possibly, Glenbrook (Glenview, IL) and Evanston Hospitals

What you will do:

  • Performs psychosocial assessments with patient and family that are consistent with age and developmental status (per department standard).
  • Communicates care issues that are identified through the psychosocial assessment to other members of the health care team both orally and in written documentation
  • Develops appropriate plan of social work interventions based on issues/problems identified through the psychosocial assessment.
  • Provides counseling to patients and their families who are experiencing problems with adjustment to illness and disability and/or issues of loss consistent with age-specific criteria.
  • Assists patients and families in crisis through emotional support, active listening and reframing consistent with age and developmental status of the patient.
  • Educates patients and families regarding rights in decision making such as advance directives and assists patients and families in advance directive implementation as requested.
  • Provides assessment, crisis intervention and referrals to protective service agencies consistent with Illinois law and as appropriate to age.
  • Educates patients and their families regarding resources in the community that can assist in maintaining patient health status and in addressing on-going psychological issues.
  • Performs discharge planning activities that are age specific and designed to best meet the patient's aftercare needs and involves patients/families as much as possible in the planning.
  • Documents all assessments, plans and actions taken in the patient's medical record per Medical Social Work Department and Medical Records standards.

What you will need:

  • Education: Masters Degree from an accredited school of social work.
  • LSW (Licensed Social Worker) or LCSW (Licensed Clinical)
  • Certification: n/a
  • Experience: 2 years post-graduate Masters degree preferred, health care or mental health care also preferred.
  • Unique or Preferred Skills: Ability to work within a health care environment. Demonstrated strong team work skills. Knowledgeable regarding state and federal laws related to abuse and neglect and the Illinois Mental Health Code. Crisis Intervention Skills. Knowledgeable about Medicare, Medicaid, and other payer requirements. Demonstrated knowledge of social work ethics and confidentiality of patient information. Demonstrated knowledge of age-specific developmental issues and the ability and willingness to work with diversity.
