This will be a Home Care CNA role in the northern part of the 13 counties we service in west Michigan. (Mason, Oceana, Lake, Newaygo)

Job Summary

Acts as a team member in the delivery of quality, direct patient care. Performs basic skills per department guidelines and communicates observations to members of the health care team.

Essential Functions
  • Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit.

  • Responsible for direct day-to-day care of assigned residents/patients including dressing, bathing, transporting and feeding.

  • Documents care provided as directed in a manner consistent with guidelines.

  • Maintains patients' environment/work area/equipment, and communicates any equipment malfunctions to the appropriate department/unit personnel.

  • Complete and maintain required number of CNA educational hours.

  • Participation in select unit quality monitoring activities under the direction of the Unit Manager or Nurse Leader.

Qualifications

Required

  • High School Diploma or equivalent

  • Completion of State of Michigan approved CNA Training Course

  • CRT-Nursing Assistant - Upon Hire

  • LIC-Driver's License - State of Michigan Upon Hire

  • CRT-Basic Life Support (BLS) - AHA American Heart Association United/Kelsey, Big Rapids, Reed City, Gerber & Continuing Care within 90 Days Or

  • CRT-Basic Life Support (BLS) - ARC American Red Cross United/Kelsey, Big Rapids, Reed City, Gerber & Continuing Care within 90 Days
