*About the Role:* As a Home Health Plan of Care (POC) Specialist, you will play a crucial role in ensuring that our patients receive the highest quality care. You will be responsible for the timely creation of Home Health Plan of Care (485) documents to meet CMS guidelines and billing requirements. Your daily tasks will include reviewing all assigned POC tasks, ensuring regulatory compliance, and collaborating with leadership to audit and analyze performance trends. *Key Responsibilities:* * Create Home Health Plan of Care (POC) documents in a timely manner to meet CMS guidelines and billing requirements. * Ensure regulatory compliance with all Plans of Care components. * Review all assigned POC tasks daily. * Assist with quarterly and team-related audit functions. * Collaborate with leadership to audit and analyze performance trends in POC accuracy. * Educate and provide feedback to clinical staff regarding the correct completion of POC documents. * Perform other duties as assigned. *Skills and Qualifications:* * Proficiency in EMR, healthcare, electronic health records, medical records software, and Epic. * Working knowledge of home health and CMS requirements for Plan of Care for both clinical and billing needs. * Ability to coach and mentor staff. * Strong analytical and decision-making skills. * Excellent written, oral, and interpersonal communication skills. * Ability to comprehend clinical policies and criteria to clearly articulate requirements surrounding health record information. * Ability to prioritize job duties to meet the demands of the day, working within tight timeframes and meeting strict deadlines. * Proactive identification of problem areas and reporting areas of concern to senior management. * Intermediate computer skills including Microsoft Outlook, Excel, Word, PowerPoint, and a willingness to learn new information technology. * High commitment to quality. *Experience Level:* Entry Level *Why Join Us?* At our company, we believe in fostering a supportive and collaborative environment where every team member can thrive. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
