Key Responsibilities:

• Community-Based Care: Provide direct support and case management services to individuals in their homes and community settings, ensuring they have the resources and skills to manage their mental health and daily living.

• Collaborative Teamwork: Work closely with a dedicated and multidisciplinary team, including psychiatrists, nurses, and peer support specialists, to develop and implement individualized treatment plans.

• Resource Coordination: Assist clients in accessing community resources, including housing, employment, medical care, and social services.

• Person-Centered Approach: Advocate for and empower clients, ensuring their voices and preferences are central to all care decisions.

• Ongoing Support & Monitoring: Help clients develop coping strategies, facilitate skill-building, and provide continuous monitoring and support to ensure sustained progress.
