Short Description Provides clinically competent case management services to clients as needed Includes phone calls, communicating with patients and families, and completing forms or letters for the physicians. Provide referrals and linkage to community-based services, and case management for patients and their families. Contribute to the development of individualized plans of care, and encourage the compliance with treatment plan goals with respect to case management objectives. Participate in daily clinical team meetings and document updates in client records. Documentation of all encounters is completed in an accurate and timely procedure within program policies and procedures. May coordinate care with outside treatment providers and care management organizations. Extract information from our EMR and utilize it to identify referral sources, time from referral to intake, and various other benchmarks. Complete reports at least monthly, working with clinical supervisor. Other duties and/or projects as assigned. Experience Required Minimum of 2 years experience in a mental health, forensic or addictions setting Education Requirements Associate degree required, bachelor's degree preferred, in Psychology or related study License/Certification Requirements BLS required. Special Requirements CPI, Handle with Care, Aggression Management certification is preferred Apply
