Job Description

Salary Range for RN $70K-82K

Salary Range for LPN $55k-$65K

Eligible for $5000 sign-on bonus

Monday - Friday, 8 am - 4 pm, Raleigh, NC, and community outreach

GENERAL RESPONSIBILITIES:

Assumes responsibility for nursing care for young adult through geriatric consumers with chronic mental illness; provides generalized and specialized care; maintains appropriate records; functions as a member of interdisciplinary team and performs related work as required. Oversees the maintenance of medical records and compliance with state regulations and CARF. Exercises independent nursing judgment.

Pre-employment Education and Experience

Qualified by an approved school of nursing program or baccalaureate of higher degree in nursing and licensed by the state, commonwealth, or territory to practice nursing. Two years experience providing direct clinical services to adults with mental illness is preferred. Newly licensed candidates will be considered after comprehensive review of training, experience, and demonstrated competence. Medical experience is preferred

ESSENTIAL FUNCTIONS AND PERFORMANCE STANDARDS:

Screening, Assessment, and Individual Action Planning

* Administers specified medication and takes vital signs as indicated and documents in consumer charts.

* Reviews referral packets and assesses referrals for medical issues.

* Provides nursing health evaluation information including assessment (if not provided), diagnosis, intervention and treatment planning information to other members of the mental health team.

* Conducts emergency consumer assessments and refers to inpatient treatment as indicated.

* Participates in treatment planning by: documenting use of clinical modality, completing assessment assignments within time requirements specified by procedures; scheduling/attending treatment planning meetings with staff and other individuals specified in procedures; utilizing assessment information, diagnostic results and staff participation in the development of treatment plans; and completing assessment, treatment plan and review documents following time and other technical specifications as indicated.

Program Development & Implementation

* Ensures medication administration is consistent with the regulations and policies of State specific medication guidelines.

* Promotes wellness within the model by providing opportunities for physical activity, broad-based nutritional education, and the treating of nicotine addiction.

* Follows all FHR policies with regard to disposal of medical waste materials.

* Ensure that the PRISM model is integrated into everyday practice and clinical interventions are being provided with each consumer.

Therapeutic Support

* Provides educational services designed to support and train consumers to self administer and monitor their own medication in accordance with their IAP.

* Provide staff and consumers with support in developing consumer's self advocacy skills regarding the effective use of medications.

* Assists consumers in the management of chronic conditions such as mental illness, diabetes, asthma, heart disease, hypertension, etc.

* Works directly in medical treatment relationship with consumers, individually or in groups, in consultation with the Psychiatrist and other physicians as indicated. Observes consumers and reports reactions to medication or treatment to medical personnel in charge.

* Educates staff and consumers and assists in consumer menu planning as indicated.

Communication & Teamwork

* Manages caseload, if indicated.

* Participates in team meetings as required.

Documentation

* Reviews consumer records to ensure that medical goals are being addressed and notes recommendations for change and new clinical approaches, as indicated.

* Follows organization procedures, including updating of consumer records, reviewing outside correspondence, and following consumer confidentiality and consumer rights procedures.

* Provides appropriate documentation regarding face-to-face and telephone contact with consumer, staff, and family members through progress notes which follow the Individual Action Plan.

* Ensures that all staff follows medication documentation as required by state specific medication guidelines.

Leadership, Coaching, & Professional Development (Leadership, Management, Supervisory, and Regional Positions)

* Conducts staff development in-services to include, but not limited to: consumer age/disability specific information, infection control, OSHA, organization policy and medical procedures, risk management, and individual staff/program development.

* Provides ongoing necessary medication training to all FHR medication certified staff.

* Exercises tact, diplomacy, and active listening in all interpersonal relations. Shows consideration and flexibility to support scheduled and emergent needs that occur within the system.

* Takes active part in formal supervision, at least once monthly.

Financial Management

* Monitor utilization of nursing equipment and supplies and oversees first aid/PPE inventories within program budget.

IV WORKING CONDITIONS ON REGULAR BASIS:

A. Necessary Travel: Travel necessary to and from meetings and Program sites.

Current state driver's license. Travel necessary to and from meetings.

Travel to physician appointments may be required.

Driving in inclement weather as needed.

B. Unusual Work Hours: Ability to respond to emergency crisis situation, if necessary. Carry a cell phone in order to be reached by Residential Director, Regional Director and COO. On call coverage as needed.

C. Unusual Environmental Conditions: Work area - inside

and outside. Environment may be noisy at times.

D. Hazards - Potential for exposure to infectious disease.

Necessity to practice Standard Precautions at all times.

Physical / Mental Requirements:
