The Clinical Risk Manager develops, coordinates and administers systems for risk identification, investigation and reduction. Collaborates with Carilion clinical leadership, Quality/ Patient Safety staff, and healthcare delivery teams as appropriate, to develop, implement, monitor, and evaluate the organization's patient safety programs. Facilitates continues improvement in healthcare safety initiatives to reduce errors and other factors that contribute to adverse patient outcomes. Serve a resource for clinical leadership and staff for risk related issues and information.
- Proactively manages assigned clinical risk management program on a day-to-day basis through the analysis of patient safety and medical error processes.
- Participates in the process of disclosure for medical errors. Works with members of the healthcare delivery team to facilitate continuous improvement in healthcare safety initiatives, reduce medical errors and other factors that contribute to adverse patient outcomes.
- Assesses institutional risks and individual adverse occurrences and near misses. Independently investigates and manages significant adverse events that occur in the clinical setting and result in patient harm.
- Follows-up with patients and families when appropriate and works proactively toward early resolution with other appropriate parties as necessary.
- Provides communication and collaborates with other departments to resolve cases that may be potential claims and provides an early warning to other departments once identified.
- Trends RCA themes, patterns; identifies/articulates opportunities for improvement to proactively manage clinical risk management. Collaborates on development of adverse event reporting and monitoring procedures and analysis of trends. Communicates with the healthcare team to identify and resolve safety issues in compliance with The Joint Commission standards.
- Acts as a resource to clinical leadership and staff on patient safety and medical error concerns, consent issues, policy revisions and reviews, confidentiality issues, standards of care practice, licensing issues, federal and state healthcare regulations.
- Active in the implementation of risk management educational programs. Promotes and assists with staff education programs to maximize patient, visitor, and employee safety.
What We Require:
Education: Registered Nurse (BSN) or Bachelor's Degree in a health sciences discipline or equivalent 8 years experience is required. Master's or other advanced degree preferred.
Experience: Minimum five years clinical experience required (recent clinical experience is preferred). Minimum three years risk management, quality management or legal nurse consulting experience preferred.
Licensure, certification and/or registration: Current Registered Nurse (RN) or other healthcare discipline licensure required. Current certification as Certified Professional in Healthcare Risk Management (CPHRM) or higher designation (FASHRM, DFASHRM) preferred. Current CPPS or CPHQ certification, or other advanced education in the field of quality and patient safety strongly preferred.
Other Minimum Qualifications: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Knowledge of national healthcare trends, multi-disciplinary professional practice models, information systems, quality management, and customer service needs. Knowledge of the fundamentals of risk management, patient safety and/or performance improvement. Knowledge of regulatory requirement, state and federal laws.
