Starting $75,000/yr total Compensation Package based on experience.

Activa Home Health (an AccordCare Company) is a licensed Medicare-certified home health agency seeking an enthusiastic customer service driven RN Clinical Manager for our Boynton Beach Office. This position will primarily work in office and will also perform field case management with responsibilities for assessment, coordination of care with other disciplines and communication with referral sources. Coordinate patient admission with other members of the care team. Must have strong case management experience and working knowledge of Medicare.

Responsibilities:
* Perform case management, oversight, and coordination of care for all skilled disciplines.
* Coordinate patient admission with other members of the health care team. Provide nursing care and treatment of home care patients in accordance with the plan of treatment and approved authorizations. RN will supervise all field staff on the case.

Qualifications:



  • Current RN Florida Professional license
  • Level II Background
  • CPR
  • Minimum of one year experience with Medicare Home Health Case Management
  • Must have excellent time management skills to prioritize and organize workload
  • Positive attitude and strong work ethic
  • Excellent communication and customer service skills
  • Experience with Kinnser / WellSky Medicare OASIS a big plus
