Full-time Case Manager Opening The Pavilion Behavioral Health System has been the leading provider of behavioral health and addictions treatment for families in Illinois since 1989. The Case Manager: provides clinical services to psychiatric inpatient and partial hospitalization (PHP) patients and their families; completes psychosocial evaluations, facilitates group therapy, individual, couple, and family therapy, and discharge planning/linkage with aftercare services; serves as a member of the interdisciplinary team; attends appropriate clinical staffing on behalf of patients and their families. Previous professional experience in a behavioral health setting preferred.
