Position Summary: The NHTD/TBI Case Manager will facilitate streamlined communication between members of the participants care team, serve as main point of contact and provide support and guidance to the back-office staff. The NHTD/TBI Case Manager will oversee participants in the Northeast region, ensuring stability and satisfaction within the guidelines of waiver services.

Job Responsibilities:



  • Focus exclusively on NHTD and TBI Waiver Participants. Serve as initial and primary point of contact for participant, family and Service Coordination.
  • Ongoing case management and oversight of participant schedules. Responsible for making suggestions aimed at increasing efficiency.
  • Ensure participant/families are notified of alterations to schedule. Deliver updated calendar to participants/family on a weekly basis.
  • Conduct weekly waiver meetings with the Northeast team (care coordination, clinical manager, and branch director). Support team in voicing suggestions aimed at achieving case stability.
  • Track progress of active SRIs - NYC, Nassau and Suffolk branches.
  • Lead monthly SRI Committee meetings. Responsible for completing minutes and delivery to appropriate RRDC representatives.
  • Responsible for completion and delivery of required waiver documents.
  • Support and attend all necessary Waiver Team Meetings.
  • Attend orientations, screen and approve caregivers for waiver assignment.
  • Respond to questions and concerns posed by participant/family. Coordinate and facilitate communication with Service Coordinator.
  • Monitor waiver schedules daily and address inconsistencies to ensure continuity of services.
  • Confirm and track approved hours upon receipt of Initial Service Plan. Ensure discrepancies are approved and noted.
  • Ensure participant and family satisfaction.



Qualifications/Requirements



  • Social Work degree
  • Waiver experience preferred
  • Physically present in Suffolk branch location - Holbrook, NY.



Pay range starting at $50,000 per year
