RHD's assertive community treatment (ACT) program in Chattanooga is searching for our next Counselor that will act as a substance use counselor and co-occurring disorder specialist. The program provides opportunities for housing support, treatment, and support services to program participants, and acts as a crucial lifeline for homeless adults dealing with frequent incarcerations, hospital visits, and ongoing crises due to severe mental illness. Job Description The counselor for RHD's Assertive community treatment program will work with multiple clients in the community where they live. This counselor position will respond to emotional crises and especially emphasize a harm reduction approach to substance use disorder. This counselor will have remarkable creative leeway given that the counselor is operating ethically, in a trauma-informed way, and can provide contemporary evidence for their interventions. The counselor will travel to within the Hamilton county area to see on average 4-5 clients per day to provider counseling, skill building, coping skills, assessments, and provide timely and accurate documentation. Mileage is compensated at the federal rate. An average day in this position - team meeting at 8:30am to review client care with multi-disciplinary team - planning day including preparing any resources needed for client care that day - in the community, visiting clients and providing interventions - paid lunch - moderating a group therapy session in the community - return to office to document and prepare for the next day On call expectation All team members who provide direct care to clients participate in a weekly on-call rotation. On call time is compensated. It is the responsibility of an ACT to provide 24/7 crisis intervention options. Position Summary Provide clinical support to program participants in the areas of addiction and mental health in either a residential or outpatient program by providing therapeutic counseling services through insight and interventions. Maintain caseload, provide both group and individual counseling services as detailed by program standards. Ensure the program supports individual goals of program participants, and that the program's recovery model is followed. Demonstrate regard for the dignity and respect of the clients and staff. Always display a caring and supportive attitude and represent the program in a positive manner. Maintain the confidentiality of all program participant information. Uphold the integrity of RHD values as outlined in the employee handbook. Therapy Partner with individuals to develop their recovery goals in relation to their medical, psychiatric and/or substance abuse disorders. Conduct individual and group counseling sessions. Assist in program participants aftercare planning and follow-up. Manage and grow regular group therapy sessions Caseload management Evaluate residents' progress and report to funders/referral sources as requested through authorization/re-authorization reviews. Participate actively in clinical case conferences as a member of the individual's support team. Maintain active caseload including engaging individuals in ongoing treatment. Maintain tracking system for treatment plans, and other required documents to meet program funder guidelines and program goals. Participate in clinical group meetings regarding ongoing treatment goals, progress and discharge as needed. Administrative Enter documentation into electronic medical records in accordance with agency guidelines. Maintain up to date scheduling calendar. Attend staff meetings and clinical supervision as scheduled Other duties assigned Requirements Qualifications Bachelor's degree from an accredited college or university in Social Work, Psychology, or a related field. OR High school equivalent with a LADAC Depending on the program license requirements, certification or licensure may be required to be obtained within a specific timeframe from the hire date. A minimum of two years of experience working with individuals with co-occurring disorders. Working knowledge of computer business software and electronic health care record programs. |
