Monday-Friday No weekends or holidays! Remote - must reside in MO, AR or OK Overview: The Diabetes Care Nurse will work in active partnership with the patient, family, provider, and clinical staff to develop and implement a well-coordinated and innovative plan of care that ensures appropriate education, goal setting, control and maintenance of their disease. This position is integral to Mercy's efforts to provide high-quality patient care and implement successful care delivery methodologies needed for alternative payment models and value-based care. Qualifications: Education: RN with a BSN or equivalent experience required. Experience: Prior work experience in Case Management, Disease Management, or Managed Care. Certification: CDCES certification preferred. Licensure: Current RN licensure in the applicable state of practice required.
