Position Summary: Participates as a member of multiple interdisciplinary clinical teams working with individuals diagnosed with a Serious Mental Illness, or General Mental Health diagnosis under Court Ordered Treatment. Provides direct services to facilitate patient access to housing opportunities, address housing permanency, and support independent living in a community setting to patients served within Terros Health Certified Community Behavioral Health Clinic (CCBHC). Works in conjunction with the patient and clinical team staff to assess patient needs and wants, assist the member in locating housing of their choice, and access affordable, safe, decent, and permanent housing. Supports patients in protecting their tenancy rights. Participates in the development of individual service plans, acts as a patient advocate, and liaisons with families and community agencies. Serves as the primary point of contact between the RBHA , inpatient settings, and CCBHC regarding housing needs for patients.

Essential Duties:



  • Conduct face-to-face interactions with patients, family, or other involved parties in the home, community, jails, inpatient settings, and/or CCBHC for the purpose of assessing, obtaining, maintaining, or enhancing a person's functioning and housing.
  • Actively participate as a part of the site's clinical teams to provide recovery supports and inspire hope for patients while supporting their achievement of service plan goals. Provides ongoing assessment and feedback on their progress in relation to their service plan.
  • Key participant in discharge planning for all patients in an inpatient, jail, or residential treatment setting. Identifies needs for housing support services, and ensures services are secured timely.
  • Participate in daily team meetings on a rotating basis; provides updates on patients housing status; identifies patients' risks and needs for further supportive housing services and work with clinical teams to coordinate care to access needed services. Offers skills training and technical assistance for site staff.
  • Responsible for transporting or coordinating transportation for patients and their identified natural supports to necessary services, as outlined in their service plan.

Qualifications

  • Bachelor's in behavioral health or related field or at least 3-5 years of behavioral health experience.
  • Three (3) years' of experience in the delivery of behavioral health services required. 3 years working as a housing specialist preferred.
  • KNOWLEDGE, SKILLS, AND ABILITIES: Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members. Good communication skills, evidenced by a non-judgmental approach, effective listening, and positive interactions. Basic understanding of the DSM-V. Knowledge of SAMHSA Permanent Supported Housing and Housing First philosophy preferred.
  • Valid AZ Driver License. Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy
  • COMPUTER SKILLS: Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred
  • Must have a valid Arizona Level 1 Fingerprint Clearance card or be able to apply for one within 7 working days of beginning employment
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
  • Must pass background check, TB test, and other pre-employment screening
