Job Summary:

In a career as a Clinical Pharmacist, you join us in providing life-changing care to patients from across the region. In this role, you will ensure safe, accurate and appropriate use of medications. Evaluate prescribers' orders for dosing, allergies and contraindications. Participate in medication ordering, preparation, dispensing, and outcome assessment.

This position is for a decentralized clinical pharmacist who serves as a key member of the multidisciplinary team. Clinical areas will include- Pediatric Intensive Care Unit, Pediatric Medical/Surgical Unit, Epilepsy Monitoring Unit, and Pediatric Inpatient Psychiatric Unit. A strong clinical background and experience with pediatrics is preferred.

Schedule: Full-Time, Day hours Monday-Friday, potential for weekend and holiday rotation

Location: Pacific Street MAJOR RESPONSIBILITIES & DUTIES:
  • Improves patient care by managing drug therapy according to the patient population served.
  • Interprets medication orders to ensure safety and proper use of medications and is alert to compatibilities, incompatibilities, medication duplication, medication allergies, and potential medication administration errors.
  • Coordinates the solving of medication related problems between pharmacy, nursing, and prescribers, demonstrating initiative and sound judgment.
  • Monitors medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions.
  • Intervenes in circumstances of significant interactions, therapeutic duplications, and other occasions where pharmacodynamics and pharmacokinetic expertise is warranted.
  • Evaluates and documents that the medication(s) selected are appropriate for achieving the desired therapeutic outcome. Communicates potential problems and likely solutions to the prescriber.
  • Participates in multidisciplinary rounds representing pharmacy on the following patient units: Pediatric Intensive Care Unit, Pediatric Medical/Surgical Unit, Epilepsy Monitoring Unit, and Pediatric Inpatient Psychiatric Unit
  • Acts as a consultant in the areas of pharmacokinetics and adverse reactions to physicians, nurses and other healthcare professionals. Participates in all therapeutic drug monitoring or intervention programs; adjusting therapy as indicated by patient condition and laboratory results.
  • Responds to emergency situations to assist with medication management (e.g. response to cardiac arrests)
  • Provides medication counseling and education to patients, families, and front-line staff.
  • Prepares and dispenses medication in a manner that ensures accuracy, safety, regulatory adherence, quality and integrity of the drug product in a timely manner.
  • Supervises the work of supportive pharmacy personnel. Carries out technician responsibilities (automated dispensing cabinet stocking, compounding, etc.) on shifts when support staff are unavailable.
  • Detects, monitors, and reports adverse drug events and medication errors.
  • Participates in the medication reconciliation process.
  • Supports drug purchaser in managing drug shortages and medication supply chain.
  • Participates in mandatory in-services and/or CE programs as mandated by Boys Town and the State of Nebraska. Completes all required trainings and pharmacy specific competencies in a timely matter.
  • Actively promotes and supports the Boys Town Mission. Fosters a healthy relationship between the department of pharmacy and the hospital. Fosters a healthy relationship within the department of pharmacy.
  • Provides patient care and education according to the patient population served.
REQUIRED QUALIFICATIONS:
  • A registered pharmacist who is a graduate of an accredited school of pharmacy with a current Nebraska state license required.
  • BLS certified within 90 days of transfer or hire required.
  • ACLS and PALS required within 180 days of hire required.

PREFERRED QUALIFICATIONS:

  • American Society of Health-System Pharmacists (ASHP) accredited postgraduate year one residency or equivalent hospital pharmacy experience preferred.
  • Board of Pharmaceutical Specialties (BPS) certification as deemed appropriate and relevant to practice preferred.
