Valley Hospital Phoenix is seeking a dynamic and talented Discharge Planner - Full Time to join our team of compassionate, dedicated professionals.

The Discharge Planner works in our Clinical Services department along with Inpatient Therapist.

Location: 3550 East Pinchot Avenue, Phoenix, AZ

Schedule: Monday - Friday

Duties and Responsibilities:

  • The Discharge Planner will assist the clinical team with coordination of care and discharge planning.
  • Responsible for having a thorough understanding of the patient's treatment through communication with the treatment team.
  • Advocate for the patient's access to services during treatment team meetings, communication with outpatient teams and referrals.
  • Meet with patients daily, be responsible for completing referrals, SMI evaluations, staffing's, and coordinating with outpatient providers.
  • Assist the team to ensure that the patient has a safe discharge placement, medication, and outpatient providers. Discharge Planners will verify safety within the home or place of discharge.
  • Discharge Planners will have knowledge of outpatient behavioral health treatment interventions, substance abuse rehabilitation, referral process for SMI services-Seriously Mentally Ill and the process of referral to ALTCS-Arizona Long Term Care, Guardianship and another other form of advocacy needed to provide excellent patient care.
  • The Discharge Planner will provide patient education by facilitating psycho-education groups.

Qualifications

Qualifications:

  • Bachelor's degree from an accredited college or university in social work, mental health, nursing, or related degree
  • A minimum of three (3) years' experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management.

Preferred Qualifications:

  • L.M.S.W., LAC, LIASC, LCSW, LMFT, LPC, or other healthcare - related licensure
  • Master's degree from an accredited college or university in social work, mental health, nursing, or related degree
